Signify Health, a healthcare platform focused on value-based payment programs, is expanding its relationship with Ardent Health Services, a provider of healthcare services with over 200 sites of care across six states. Ardent, a former participant in the Center for Medicare and Medicaid Innovation’s Bundled Payments for Care Improvement-Advanced program, will join a Signify Health-enabled accountable care organization (ACO) in 2023 to enhance care transformation, risk management, and the delivery of coordinated care to its Medicare patients.
Ardent’s affiliated facilities and physicians in New Mexico, Oklahoma, Texas, and Kansas will collaborate with independent providers in an ACO model to achieve scale benefits and effectively manage risk. This strategic partnership supports alignment with the Centers for Medicare & Medicaid Services’ (CMS) goal of enrolling all Medicare beneficiaries in an accountable relationship by 2030.
Signify Health recently acquired Caravan Health, a leader in enabling sustainable ACO and population health programs. This acquisition strengthens Signify Health’s position as a platform with one of the largest networks of at-risk providers and a comprehensive solution to manage the total cost of care. In 2020 and 2021, 100% of Signify Health’s collaborative ACO participants earned shared savings.
According to Tim Gronniger, Chief of Value-based Care Solutions at Signify Health, this collaboration will provide Ardent’s Medicare patients with increased access to preventative and in-home services that can help reduce health inequities and generate cost savings.
